健康保険法施行規則第50条及び厚生労働省の指導に基づき、被扶養者資格確認調査（検認）を
実施いたします。
被扶養者の削除漏れ等、本来扶養から外れる方が含まれていた場合、保険給付費や健康保険組合の
拠出金等にも影響を及ぼすことから、被扶養者認定状況の確認にご協力をお願い申し上げます。
本調査は、株式会社法研に委託しております。

Based on Article 50 of the Ordinance for Enforcement of the Health Insurance Act, we are
conducting the survey to review eligibility of dependents.
As we need to keep the financial situation clear, we are asking those members who have
registered dependents with the Health Insurance Association, to update the status of their
family members. We appreciate your cooperation.
We outsource this survey to Houken Corporation.
