健康保険法施行規則第50条及び厚生労働省の指導に基づき、被扶養者資格確認調査（検認）を
実施いたします。
被扶養者の削除漏れ等、本来扶養から外れる方が含まれていた場合、保険給付費や健康保険組合の
拠出金等にも影響を及ぼすことから、被扶養者認定状況の確認にご協力をお願い申し上げます。
本調査は、株式会社法研に委託しております。

１）2023年4月1日時点で20歳以上 の被扶養者で、当組合が各自治体等に照会した2023年収入が以下の基準に該当する方 【基準】 60歳未満の場合は、130万円以上 60歳以上または障害年金受給者の方は、180万円以上 ２）各自治体等に情報照会ができなかった方 |
| 回答方法 | 対象者がいる被保険者のご自宅宛てに、委託先（株式会社法研）から「健康保険被扶養者調書」を郵送しております。ご回答を調書にご記入の上、同封の返信用封筒にてご郵送ください。 |

Based on Article 50 of the Ordinance for Enforcement of the Health Insurance Act, we are
conducting the survey to review eligibility of dependents.
As we need to keep the financial situation clear, we are asking those members who have
registered dependents with the Health Insurance Association, to update the status of their
family members. We appreciate your cooperation.
We outsource this survey to Houken Corporation.

| Procedure | The survey report（健康保険被扶養者調書） will be sent by the outsourcing company, Houken Corporation, to the home of the insured person who has family members to be verified. |
