【Important】Regarding sending "Notification of Eligibility Information"
Regarding sending "Notification of Eligibility Information" and request for confirmation of your Individual Number
Due to the change in the law, the "Individual Number Card" and "Health Insurance Card" will be integrated, and after December 2, 2024, "Health Insurance Card" will no longer be issued.
* Even after the discontinuation of Health Insurance Cards, transitional measures will be in place to allow Health Insurance Cards already issued to be used as usual for up to one year (until December 1, 2025).
In accordance with this, based on a notification from the Ministry of Health, Labour and Welfare, a " Notification of Eligibility Information " will be sent to the insured person's home address by mail (Registered Mail).
We will also send to the insured person enclosed dependents' " Notification of Eligibility Information”.
The purpose of sending you the " Notification of Eligibility Information " is to inform you of the health insurance eligibility information registered with our association, as well as to confirm that your "Individual Number" has been registered correctly so that you can use your "Myna Health Insurance Card" with peace of mind.

【1】Request for Confirmation of your health insurance eligibility
The "Notification of Eligibility Information" we will send you will contain the last 4 digits of your "Individual Number " registered with GE Health Insurance Association.
Please confirm that there is no mistake with the last 4 digits of your “Individual Number”.
If the last 4 digits of the “Individual Number” listed do not match the last 4 digits of your Individual Number, please contact us.
If the last 4 digits of the Individual Number listed match the last 4 digits of your Individual Number, you do not need to do anything with this document.

【2】Please keep hold of your "Notification of Eligibility Information"
After December 2, 2024, "Notification of Eligibility Information" card may be needed when seeing a doctor at a medical institution.
Additionally, you will need your health insurance code and number when applying to GE Health Insurance Association, please keep it in a safe place.

■ How to see a doctor on or after December 2, 2024 ■
Due to a change in the law, new Health Insurance cards will no longer be issued after December 2, 2024.
★ Transitional measure for using "Health Insurance Card" ★
Even after the discontinuation of Health Insurance Cards, transitional measures will be in place to allow Health Insurance Cards already issued to be used as usual for up to one year (until December 1, 2025).
(1) For those who have an “Individual Number Health Insurance Card (Myna Health Insurance Card)”:
By registering your Individual Number Card as a Health Insurance Card, you will be able to use your Individual Number Card* to visit medical institutions.
Notes:
“Myna Health Insurance Card” can only be used at medical institutions that have adopted the online eligibility confirmation system. If you visit a medical institution that have not adopted that system, you can receive medical treatment in the following ways.
① You can receive medical treatment by presenting your “Notification of Eligibility information” card with your “Myna Health Insurance Card”.
② You can receive medical treatment by presenting your individual number portal* health insurance eligibility screen on your smartphone with your “Myna Health Insurance Card”.
* Individual Number Portal is called "Myna Portal".
(2) If you do not have "Myna Health Insurance Card", you can receive medical treatment in the following ways.
■ For those who have a “Health Insurance Card”:
Your current “Health Insurance Card” can continue to be used until December 1, 2025. After December 2, 2025, you will no longer be able to receive medical treatment using your “Health Insurance Card”.
■ For those who do not have "Individual Number Card" or have an "Individual Number Card", but have not registered for use as a "Health Insurance Card":
You can see a doctor by presenting the "Certificate of Eligibility Card" that GE Health Insurance Association issued.
Those who do not have an Individual Number Card or who do not register an Individual Number Card as Health Insurance Card will be issued a "Certificate of Eligibility Card” by GE Health Insurance Association.
The application procedure for a “Certificate of Eligibility Card” is currently under preparation.
