Based on Article 50 of the Ordinance for Enforcement of the Health Insurance Act, we are conducting this survey to review eligibility of dependents.

The Dependent Eligibility Survey form has been sent to you by postal mail.  Please confirm the contents and documents required for submission.  Attach the documents to the form and using the envelope provided in the mail, send them back to the GE Health Insurance Dependent Eligibility Survey office.
