GE健康保険組合　平成26年度健康診断実施概要について

１．対象者：GE健康保険組合に加入する被保険者および被扶養者

２．受診予約期間：平成26年9月1日（月）～ 平成27年3月31日(火)

３．受診期間：平成26年10月1日(水) ～ 平成27年3月31日(火)   

＊注１：健康診断は通年（4月1日～翌年3月31日）で行われますが、今年度に限り

  9月1日から予約が開始されます。

＊注2： 健診の種類、費用負担額等は順次ご案内いたします。

＊注3：社員の方（被保険者）は、8月に会社から配信される健診案内をご参照ください。

FY2014 Health Examination Process Overview　

1. Eligibility: All employees insured by GE Health Insurance Association and their dependents.

2. Reservation Period: Sep1, 2014 (Mon) - Mar. 31, 2015 (Tue)

3. Health Exam Period: Oct. 1, 2014 (Wed) - Mar. 31, 2015 (Tue)

Note 1: Health examinations are usually held from April 1st through March 31st of the following year, but for this year only, reservations will be accepted from September 1^st .
